Does it works on guitar too or only on bass?

does it come with the blend and volume pot too or just the bass mid and treble?

So this comes with individual treble, mid and bass pots. Can I add my own blend, volume and mid DPDT switch when wiring up?

Please help I just received my pre-amp and will do installation myself. My pre-amp did not have a battery connection with it?
