It uses Velcro and opens on the side Put your chin in the chin pad and pull it tight to your comfort level and use the knob on the chest piece to go up and down for tension adjustments

Q: can you sleep in this device?

I know I couldn’t, I use it for therapy before going to bed, which really does help to relieve pressure from nerves, and it allows me then to sleep better, but not with keeping it on for sleep. It really does an excellent job!!
