Do I need one of those gun things to squeeze it out, or can it be squeezed out by hand?

You can squeese it out by hand by using something like the back of a hammer. It will be easier to control and simpler to dispense by using a quart size caulking gun.

What size nozzle do I need for this??

This comes with a nozzle. Both, the tube and the nozzle need to be cut. You need a quart size caulking gun.

I have a door frame that is old and has cracks and crevices all around. Will filling those with this make a difference or can I use regular cauilk?

Actually this caulk is meant to reduce the noise that can come through cracks such as around the edges of doors, drywall seams where no stud is, etc. This is not used between two layers of drywall as the previous answer states. The green glue sealant (not called caulking) is used between two layers of drywall.

Does it come how it looks on the picture??

The graphics on the tube have been slightly redesigned (Amazon is not showing the most recent uploaded image yet). The nozzle on the top has to be screwed on once you receive it.
