I didn’t get the installation guide included, how can I find out how to install?

I did not get it either not did I get any mounting screws

I purchased these 48" slides two months ago, they are great. All of a sudden, it will not lock when in. It locks out, but not in. Any suggestions ?

I would check to be sure there isn't an obstruction in the track itself. The drawer does have to go All the way back before it will lock. Is there anything behind it? Anything that's fallen into the track of the guide?
