Q: Does the l/xl run smaller or bigger? the person's waist size is 52" and i wanted to know if i should get the l/xl or 2x/xxxl size.

I purchased a 3x and found that was a little large for me. I had purchased it for post-op care. Remember you can always return the item if the size is not right.

Q: How long can you wear this before you should take it off to let your body breathe/relax? And which usually works better, day or night?

The Abdominal Binder shouldn't be uncomfortable, if you feel uncomfortable you should take a rest to relax as you suggest. I would also recommend increasing the duration of wear over time, to allow your body to be used to wearing the stomach binder.
