When the rail is folded up, what is the distance between the wall and the first rail? (I'm trying to determine if it will clear the windowsill above.)

Basically one inch (2.54cm) though there is some leeway since it doesn't have to go all the way up to stay put. Alternately you could put a spacer behind it before screwing it to the wall to gain more clearance.

Can this grab bar be used with 45” toilet?

It can be used with most any toilet it mounts to the wall based on tge height of the toilet but can put at any height u want .just mount it properly
