Does it really massage your eyes (eyeballs) or is it just the surrounding areas?

Around the actual eyeball but you can feel decent pressure. Mine lost its ability to heat up so that was very disappointing to me. I really wanted it for the heat, which you could feel on your eyeballs through your eyelids. Now it just sits in a box.

How does this heat up exactly? Is it safe to use the heat. Are there any long term use effects to the eyes?

It doesn't get very warm even on the highest level. It is similar to a warm compress on your eyes. I can't speak to the long term effects because I am not a medical expert but I personally have no concerns about the safety of using it occasionally. I am not certain what is being used to heat it up, but I would imagine something similar to a heated blanket. The heat is even and I have experienced no hot spots or temperature differential, the heat is mild, relaxing, and just feels nice.

Does anyone use this on a daily basis? Have you seen and felt improvements?

Don't use it on a daily basis, but when we do, there is definitely an immediate gratification. My husband and daughter suffer from ocular migraines. At the onset of one, they use this and it helps immensely.
