To take a card out would you have to unclip the badge holder from the lanyard or whatever you attach it to?

Yes, the holder has the cutout on both the front and back that the clip's strap loops through. Cards are secured in place this way. The clip's strap has a snap on it that allows it to be disconnected from the card holder easily.

What are the dimensions of the card slot? Or, how big of a card will fit in this holder?

I put in a 2x3 size id card. It still had a little room to adjust up and down. All the card info was within the window with nothing blocked. I only used mine as a Halloween prop. Hope it helps.

Does it connect easily to a keyring retractable leash?

I suppose it could depending on whether the leash has a place to attach the tags spring clip.
