Rubber or plastic mouthpiece ?

Plastic. It's one of my favorite student mouthpieces. Hite is another good one. Yamaha 4C is easier for the youngest players. (Yamaha 3C worked for my student with a pronounced overbite.). However, I prefer my private alto students play on the Selmer S80 C* and recommend it to most of my Jr. Hi and HS students unless they can't afford it.

Does this come with the reed and the piece to screw it down?

It doesn’t come with a reed or ligature clamp. Clarke Fobes can supply you with a variety pack of reeds and ligatures are of many types. For my students I liked the Luyben clarinet ligature.

What is the tip opening and its chamber size?

The tip opening is 1.00mm and is a medium chamber.
