Can this unit be used as a High Voltage solution? I would like to put 2 rececpticles on either side and then a coaxial connection in the middle

Yes, that is exactly the way I have mine installed and it works great!  Be aware that it will become a Love/Hate relationship with this thing. You will love what it does and how it looks but hate what you have to do!  First, if you have 16" C.T.C. studs, the opening must be cut EXACTLY in the center - any mistake and you will be repairing your drywall.  This is because using two metal outlet boxes at each end will cause the back of the fixture to be about 3" wider that the front faceplate - so wide, in fact that it barely fits between the studs.  This makes inserting the finished and assembled unit into the opening nearly impossible ... something like one of those aggravating Chinese puzzles!  What worked best for me was to mount one metal box on the left side (a box with the small drywall wings tightened with a single screw at each side).  For the other side I used the type of box that has the "U" shaped spring clip (big one) that snaps open after insertion and holds the box tight against the drywall - except in this case it would hold it tight against the plastic fixture.  I first inserted the unit into the opening at the left side of the hole.  I next ran the cable through the mounting hole at the right side and inserted it into the metal outlet box.  the outlet box was not yet mounted in the fixture but I needed to do this as the hold-down screw for the cable strap is at the OUTSIDE of the box and can not be tightened once the box is inserted into the wall.  After securing the cable to the box, I then shoved the rest of the fixture into the opening and then pushed the metal outlet box into the fixture opening until it snapped in.  Only thing left to do was to tighten the single screw at the bottom of the box which holds the "U" spring tight.In my community, our electrical code does not allow using plastic boxes for high voltage .... at least not the ones that are supplied with the unit by P&S.  Only ONE (1) plastic box comes with the unit.  Instructions tell you that other accessories are available but do not hold your breath on finding any!  The P&S web site allows you to search for local distributors in your area.  After contacting 11 in my area, I could not find even ONE who carried the accessories for this unit!  Why?  I was told that P&S requires their distributors to buy items in quantities of 100 minimum.  This item is so rare that they expect to sell only 2 or 3 per year!  In other words, in the next 40 years, the fixture will be obsolete and the accessories will still be on the shelves.One last word of warning!  Measure your opening carefully ... the fixture is held in place by 4 screw type clips - two at the top and two at the bottom - each clip is only 3/4" long so if you cut an opening too large the clips will have nothing to hold on to.  I cut my "second" opening 1/8" smaller than my measurements all the way around and enlarged it with a drywall rasp to get a good and tight fit.  You will have fun with this item - it will certainly help to enlarge your vocabulary of four letter words.  Nevertheless, this is one extremely attractive, sturdy and functional unit and I would highly recommend it to anyone!  Make sure your TV wall mount is well oiled as you will be pulling it away and back often when everyone who sees it begins to ask why there are no wires hanging down the wall   :)

Does this one really come with a surge protector? There is a very similar product like this one being sold on Amazon (made by On-Q/ Legrand). It also has a photo with a surge protector outlet, but it doesn't really come with one according to the customers who have purchased one.

This definitely does come with one. The other SKU is missing the surge protector & the rework box it goes with. I actually purchased the basic package first & then had troubles locating the rework box & ended up purchasing the full monty kit here to make sure everything fit/works. I had a leviton hospital-grade surge protector on hand already so I can't comment on the quality of the included surge protector, but the kit itself is really awesome.
