heel to toe difference? I need an elevated heel

It’s about the same, toe to heel, but you could add an insert or orthotic for that to make it higher in the heel.

Do they fit true to the shoe size?

I wear 11.5 these fit perfectly. Nice and snug, however I have a very high arch so I added an 1/2 insert from goodfeet most comfortable shoe/boot I own better for my knees.
