Will this hold a canon 5d mark iv?

I'd say yes. It's rated to hold up to 90 kg (200 lbs). The bottom plate screws into your camera (like a tripod plate would) and can be adjusted to sit further back or forward so it doesn't interfere with pull- out screens. Then you just have to scream the clip to something solid - I have mine on the left hand strap of my backpack - but I also have the PROPad which lets you attach it lower on belt (kind of like a holster). You could also just put it straight onto your belt but I find that difficult to clip and unclip (it's just a weird place). I use mine almost every day with a SONY a7riii, admittedly that is a mirrorless so reasonably light body - but with a 135mm sigma lens on, the weight soon builds up. It manages fine.

is this new version metal? i have the old version which in cold weather the camera was difficult to get in and out of the strap. anyone have this  too

Yes, this one is all metal, works well, although getting the camera in and out can sometimes be a little tricky. You need to develop the knack, it takes a little practise. It is extremely well made/engineered, I go long-distance mountain hiking with it and carry a large SLR with heavy zoom lens, so  it can take some weight also.

Would this work with the CL-3 handstrap attached?

I can't say for certain it would fit with the strap attached. I have mine attached to my camera bag strap.

Question 4

What is the difference in this plate & the dual plate? trying to work out the best option, this set or buy both clip only plus the dual plate

Hi Nick, the Dual Plate is compatible with Manfrotto RC2 tripod heads AND ARCA heads, BUT only goes into the Capture Clip in two, rather than 4 ways.
If you don't have a Manfrotto Tripod, it is best to go with the Standard Plate.
