Q: will this belt fit a 54 inch waist?

No it fits Max at 50 inches I bought this and made that mistake just as a heads up also as a recommendation if you really need a tactical belt I recommend the Fairwin tactical belt it's on Amazon for about 17 to 20 dollars and they have different sizes you can choose from including 54 inch just it doesn't come with the holster and other items

Starfleet would not be caught dead using a duty belt like this....they don't even use duty belts, just pouches

Q: Will this adjust to fit a child?

Most likely no about fitting a child depending on the age. Also, it's gonna be a little heavy in the back depending on how much equipment you are gonna have on the back of the belt. Prefer getting belt keepers just in case.
