By one size higher then your waste size
Do not go by size chart . I have a 30W after one washing they shrunk. Unable to use them.

Works well at keeping it all it - Use mesh bag and change them every day
I am too young to need to wear these things, but as the saying goes "it is what it is." My pelvic floor has its own agenda and my condition should not be permanent. I have been wearing these "super" briefs for a couple months now. I will tell you all I think you need to know man to man. These briefs keep everything in: sweat and #1. They are on the tight side on purpose to prevent spills. That being said, you will definitely want to change these things everyday. You will usually smell an intense odor when taking a #1 or #2. If you are extremely active, I double down on my odor warning. Additionally, your #1 may feel warmer than normal after physical activities. Your pad in the brief is doing its job. Its absorbency pad is taking it all in: heat, #1, and sweat. I am sure it is the same for any pair from anyone you buy. You will probably find these things tight. It takes some time to get used to. The company says they'll exchange up a size if you are too uncomfortable. I have been tempted just to try the larger size to see if they breathe better. I had an issue with one pair of my briefs. Wearever, the company that sells these briefs resolved the issue promptly. It could be one-time bad luck, but if you have a mesh laundry bag, I recommend you throw these briefs in the mesh bag when washing and drying them. My wife has one for her bras. Now these super briefs go in there for protection and longevity. Now, chin up, swallow your shame and make the purchase. You are far from alone in wearing these things.

They leak
They leak. I had to urinate thinking there would be no leaks. Dead wrong. When I sat down, the urine leaked through the briefs, my pants and onto the chair. Sorry I bought them, would not recommend. The disposables do a better job.

Doesn't absorb quickly enough
Since my radical prostatectomy I have had a dribbling issue. I had high hopes for this product as I am concerned about throwing adult diapers and pads in the landfills. I am disappointed. Every time I wore these they leaked all of the way through to my clothes. It seems that the smallest of leaks by-passed the padding and soaked right into the brief and on to my pants. Whatever the material that is used in these "absorbent" pads is appears to be urine proof.

Good fit
My husband loves these. We’ve purchased a dozen pair. Size is true to fit. I do use laundry bags to wash them in as the elastic waistband can unravel.

says 38-40 compared to other large briefs seems over sized. Good material and heavy duty pad

Not sized properly. If you are between sizes this will not work for you.
