Q: Do they flatten out over time, or do they bounce back?

Not in my experience. I have replaced all of the pillows in our home with these, and have had no issues with flattening.

Q: Would these pillows work for a back sleeper?

I am a back sleeper and this pillow I use stacked on top of a more firm king size pillow and it gives the perfect comfort to my neck and back. We love these pillows!
