Q: can rod be adjusted longer by turning when it is on the wall to make a tighter fit?

We had no problem with adjusting it for a tighter fit. We can even hang wet towels over it and it remains secure. I’m happy with this product.

Q: Do the shower curtain rings roll smoothly in the middle where the rod adjusts? The end caps look thick, does this cause a gap ?

The end caps do stick out about an inch and a half but my shower liner still has plenty of width to adhere to the side walls of the stall and prevent leakage. The rod is telescoping so where the smaller diameter rod fits into the outer rod, there's a small bump due to the different diameters. The rollers on the curtain holders due tend to get stuck slightly at this joint but not badly. Hope this helps.
