my Dad is 275 and he used this cushion though it did flatten it out a bit over time

Pain relief, I'd say it probably does somewhat provided they are not to heavy. The person using it here sits and lays back a lot in the chair, I bought it primarily to help prevent bedsores. And for that it has been phenominal. So for me it provides pain relief because it helps keep the bedsore away.

Q: Can you use this on couches?

Yes. I purchased this for my Mom and she uses it all the time, especially in her recliner where she sets 95% of the time. She had purchased a couple of other cushions before with the monetary aspect as the guidance only. Much to her dismay. She loves this cushion and uses it all the time. It does not scoot around in here chair, might have to be a little more aware on a couch when adjusting your seat position.
