where is this wrench made?

It looks similar to some of the ones found at Lowes but the feel and quality between the two wrenches are a huge difference. High quality wrench. Love it!

Does it come with a case?

As of 01-10-2020 it does come with what appears to be a very nice and solid black poly case.

Does dual scale mean both Ft/Lb and Nm scales are marked on the wrench?

Mark, that's exactly what it means. There is one scale on one side of the wrench and another scale on the other side of the wrench. It lets you adjust the wrench to what ever specs apply to the machine your working on. I work on a lot of french made helicopters. All the manuals designate metric torques (Nm)

How accurate are they at 95 ft lbs or 15 ft lbs?

The CDI 1503MFRMH is no longer being calibrated at 20ftlbs, scale no longer starts at 10ftlbs, changed to 20ftlbs and calibration now starts at 30ftlbs, maybe to keep people from using the 10ftlbs setting that may have been to fare off, who knows?
