Q: It says sizes small, medium, large and xl. Which do i pick for my shoe size 10.5?

Hi, The size range is in the bracket beside the non-numerical size. A large would work but so would a XL as it can be trimmed down to a 10.5

Q: are they comfortable if have high arches?

They have a low to standard arch height. The steel is not moldable. If you like support for your arch, I wouldn't recommend.
