The instructions warn against over-tightening.What can be done to keep keys on the bottom half from flopping out of the KS and pointing at the ground?

Tighten it. Ever couple of weeks I have to tighten it up a quarter turn or so. Just enough to make it take a little effort to move the keys. I never let my keys get loose enough just to flop out.

What's the difference b/n Compact & Extended in terms of the length?

The Standard/Classic KeySmart fits keys up to a maximum length of 53mm, whereas the KeySmart Extended will fit keys up to 80mm.

Do standard keys fit well with the extended version? For example I have one extended key but most are standard size. Is that a problem?

The KeySmart Extended can accommodate any length of key up to 80mm. So yes, even standard sized keys will fit well in the Extended version. When in doubt, go with the Extended version :)
