When checking a 3 phase motor how does one hook up to the leads?

You would have to test each phase (wire) individually, as a megger is meant to test one single conductor at a time. The (E) lead is your ground and your (L) lead is attached to the conductor you would like to test. Any reading other than basically zero resistance is usually bad.

So how does this differ from the more $$ fluke and megger brands? i have a fluke 87 but need to test insul on some 1/0 wiring i pulled in my house.

It doesn't doesn't come with the name recognition (and MSRP) of Fluke, nor does it come with the same build quality, overall performance or certifications of the name brands. If you need a meter like this for every day professional use go with a name brand. For occasional use by hobbyists / DIY'ers / homeowners, this one will do the job.

Correct. The same outer case is used for different types of meters that might need the extra contacts and more batteries. Just a cost saving measure for the manufacturer.
