As of December 2021 have the packaging issues that many have reported been resolved?

It really depends on the company selling it. We carefully package our ornaments to reduce the risk of breakage in transit, but some companies do not.

do these lights come with the transformers? if not what transformer is required?

These were cheap glass ornaments to put on Christmas tree - it does not light up! I didn't like the quality of the ornament.

How do you affix this to the dollhouse ceiling?

I have a woman's sterling silver chain connected to it and use it on my mirror in my car. It's for a Christmas tree but you could use yarn and hook it up anywhere.
